How to Apply
Please find below a convalescent application form for the 2024 season, along with the GDPR form (General Data Protection). Please print and return BOTH to the Pontefract Office.
If you are new to Lynwood and have not attended previously you must supply one piece of verification to prove your link to the mining industry e.g. Mineworker's Pension Scheme pay slip or letter, training records, an old payslip or any other official documentation. If you have any trouble locating any of the above documents then you will need to apply to IRON MOUNTAIN to release the employment records. Please call them direct on 01785 218030. Once you have received your records from Iron Mountain, these must then be forwarded onto the Pontefract Office as soon as possible to complete the verification process

We only require 1 application form per year.

If you wish to attend more than once in a calendar year, please either specify your cancellation preference on your form or contact the office to be put on the cancellation list.
